Information we collect

• Information you give us. When you register as a patient, refer a patient, call us or visit, we may collect your name, contact details (including your mobile number), date of birth, dental and medical history, insurance details and payment information.
• Through our secure portals. Online registration and referrals are handled through secure portals operated by our practice-management provider on our behalf. Information you enter there goes directly into our patient records.
• Basic technical information. Like most websites, our hosting provider automatically processes basic technical details, such as your IP address and browser type, to deliver pages and protect the Site from abuse.

The Site does not use analytics, advertising pixels or tracking tools, and it does not set cookies of its own. The portals may use cookies needed to keep you signed in securely.

How we use it

We use the information we collect to:

• schedule appointments, provide care and communicate with you and your referring dentist;
• verify insurance, file claims and process payments;
• send appointment, care and account messages you have agreed to receive;
• keep the Site and our systems secure and working properly; and
• meet our legal and professional obligations.

We do not sell your personal information.

Text messaging

If you opt in to text messages, we use your mobile number to send messages about your appointments, care and account. We do not send marketing text messages. Message frequency varies, and message and data rates may apply. Reply STOP to opt out or HELP for help at any time. Full program details are in the Text messaging terms of our Terms of Service.

No mobile information will be shared with third parties or affiliates for marketing or promotional purposes. Text messaging originator opt-in data and consent will not be shared with any third parties, except service providers that deliver messages on our behalf.

When we share information

We share information only as needed to run the practice and care for you, including with:

• your referring dentist and other providers involved in your care;
• your dental or medical insurance, to process claims;
• service providers that support our practice-management system, patient portals, messaging and payments, who are required to protect it; and
• authorities, when the law requires it.

Health information is shared only as described in our Notice of Privacy Practices.

How we protect it

The Site is served over an encrypted (HTTPS) connection, and our portals use secure, access-controlled systems. Access to patient information is limited to team members and providers who need it. No method of transmission or storage is completely secure, so please use the portal, phone or an in-person visit, not regular email or text, for detailed health information.

Links to other sites

The Site links to services run by others, such as our secure portals and Google Maps for directions. Their own privacy policies apply when you use them.

Children

The Site is not directed to children under 13, and we do not knowingly collect information from them online. A parent or guardian provides registration information and consents to text messages for patients under 18.

Your choices

You can opt out of text messages at any time by replying STOP. To update your contact information or ask a question about your information, call us at (480) 310-2000. Your rights to access and amend your health records are described in our Notice of Privacy Practices.
