Website information is not dental advice

The Site is here to help you understand endodontic care and our practice. It is general information, not a diagnosis or a substitute for an examination by a dentist or endodontist. Reading the Site, using a portal or exchanging messages with us does not by itself make you a patient of the practice. That relationship begins when you are seen and accepted for care.

Urgent concerns

Please do not use the Site, the portals, email or text messages for anything urgent. If you have severe pain, swelling or a dental injury, call our office at (480) 310-2000 and tell us you are in pain. For a life-threatening emergency, call 911 or go to the nearest emergency room.

Patient information, portals and appointments

You may give us personal and health information through our secure online patient portal (for example, new-patient registration and health history), through our referring-doctor portal, by phone or in person. When you do, you agree that:

• Your information is accurate. Your contact details, dental and medical history, medications, allergies and insurance information are accurate and complete to the best of your knowledge. Your care depends on it, so please tell us promptly if anything changes.
• You are authorized to provide it. You are the patient, or the patient’s parent, legal guardian or legally authorized representative. A parent or guardian must complete registration for a patient under 18. Referring providers may submit patient information and images only as HIPAA and applicable law permit.
• We protect it as health information. Health information you give us is handled as described in our Notice of Privacy Practices. Our patient and referring-doctor portals are operated by a secure third-party practice-management provider acting on our behalf, and your use of a portal may also be subject to that provider’s terms.
• You use the secure channel for health details. Please share health information only through the patient portal, by phone or in person. Regular email and text messages may not be fully secure.
• You keep your login private. You are responsible for keeping your portal login confidential and for activity under it. Tell us right away if you believe someone else has used it.
• We may contact you about your care. By giving us your contact information, you agree that we may contact you by phone, email or mail about your appointments, care and account. We send text messages only if you opt in, as described below.

Appointments

An appointment is scheduled only once our office confirms it with you. Patients who are in pain are prioritized, and we make every effort to see urgent cases promptly. Because urgent endodontic cases can come up during the day, appointments occasionally run late. We appreciate your patience.

Payment and insurance

As a courtesy, we prepare and submit claims to your dental insurance. Insurance estimates are not a guarantee of coverage, and you are responsible for any portion of your bill that your insurance does not pay. We review your estimated portion with you before treatment begins, and our office financial policy is provided to you separately. If we offer online payment, it is processed by a secure third-party payment processor, and card numbers are not stored on the Site.

Text messaging (SMS) terms

Program

Program name: Heaven Hill Endodontics Patient Messages.

What we send: If you opt in, Heaven Hill Endodontics may send you text messages about your care and your account, such as appointment confirmations and reminders, scheduling and rescheduling notices, requests to complete patient forms, pre-operative and post-operative instructions, billing and payment notices, and replies to messages you send us. We do not send marketing or promotional text messages.

Opting in

You can opt in by giving our office your mobile number and agreeing to receive text messages, for example on our patient registration forms or by telling our team. After you opt in, we may send a one-time message confirming your enrollment. If you are under 18, a parent or guardian must consent on your behalf.

Consent is optional. Agreeing to receive text messages is not a condition of receiving treatment or purchasing any goods or services.

Frequency and cost

Message frequency varies based on your appointments and account activity.

Message and data rates may apply. Check your mobile plan for details. Heaven Hill Endodontics does not charge you for text messages.

Opting out and getting help

You can cancel at any time. Reply STOP to any message. We will send one final message confirming that you are unsubscribed, and then you will receive no further text messages from us. To start again, reply START or call our office.

Carriers and your number

Mobile carriers are not liable for delayed or undelivered messages. Delivery depends on your mobile carrier and is not guaranteed. The service is available on major U.S. wireless carriers and may not work with every device.

Please tell us promptly if you change or deactivate the mobile number you gave us, or if your phone is lost or stolen. You confirm that you are the account holder or an authorized user of that number.

Health information in texts

Text messaging is not a fully secure way to communicate. Our messages contain only the information needed for their purpose. Please do not send detailed health information by text. Call us instead.

Privacy

No mobile information will be shared with third parties or affiliates for marketing or promotional purposes. Text messaging opt-in data and consent will not be shared with any third parties, except service providers that deliver messages on our behalf. See our Website Privacy Policy for details.

We may change or end the text messaging program at any time. Changes to these SMS terms will be posted on this page.

Using the Site

You agree not to:

• use the Services for anything unlawful, or give us false or misleading information;
• submit information about another person without authorization;
• try to gain unauthorized access to the Site, the portals or our systems, or disrupt how they work;
• copy, scrape or reuse Site content for commercial purposes without our written permission.

The text, images, logos and design of the Site belong to Heaven Hill Endodontics or are used with permission, and are protected by copyright and trademark law. You may view and print pages for your own personal, non-commercial use.

The Site links to services run by others, such as our secure portals and Google Maps for directions. We do not control those sites and are not responsible for their content or practices.

Disclaimers and limitation of liability

The Site is provided “as is” and “as available.” We work to keep it accurate and available but cannot promise that it will always be complete, current, uninterrupted or error-free. To the fullest extent permitted by law, we disclaim all warranties, express or implied.

To the fullest extent permitted by law, Heaven Hill Endodontics and its doctors, team and service providers are not liable for any indirect, incidental, special or consequential damages arising from your use of the Site, the portals or text messaging. Nothing in these Terms limits liability that cannot be limited by law. These Terms do not govern the clinical care you receive, which is governed by professional standards and applicable law.

Changes and governing law

We may update these Terms from time to time. The “Last updated” date above shows the latest revision, and continuing to use the Services after a change means you accept the updated Terms.

These Terms are governed by the laws of the State of Arizona. Any dispute about them will be handled in the state or federal courts located in Maricopa County, Arizona.
